When completing road locations/reconnaissance a RECON REPORT is usually completed. In its most basic form it contains the "raw field notes" (see sample field notes). Usually the field notes are updated in the post-field phase with additional comments/observation providing more detailed information than is contained in the raw field notes. Nowadays an additional aspect is that such a report creates an electronic document and is therefore much more easily retrievable than hardcopy field notes, stashed away in some drawer or shoe box.. Adopt a comparable format for your report. Scan the field note pages and include them to the word document as an appendix.
